Introduction Future HIV vaccine efficacy trials with adolescents will need to ensure that participants comprehend study concepts in order to confer true informed assent. A Hepatitis B vaccine trial with adolescents offers valuable opportunity to test youth understanding of vaccine trial requirements in general. Methods Youth reviewed a simplified assent form with study investigators and then completed a comprehension questionnaire. Once enrolled, all youth were tested for HIV and confirmed to be HIV-negative. Results 123 youth completed the questionnaire (mean age=15 (...) years; 63% male; 70% Hispanic). Overall, only 69 (56%) youth answered all six questions correctly. Conclusions Youth enrolled in a Hepatitis B vaccine trial demonstrated variable comprehension of the study design and various methodological concepts, such as treatment group masking. (shrink)

Whether ethics is too important to be left to the experts or so important that it must be is an age-old question. The emergence of clinical ethicists raises it again, as a question about professionalism. What role clinical ethicists should play in healthcare decision making – teacher, mediator, or consultant – is a question that has generated considerable debate but no consensus.

We present a new road map for research on “How the Brain Got Language” that adopts an EvoDevoSocio perspective and highlights comparative neuroprimatology – the comparative study of brain, behavior and communication in extant monkeys and great apes – as providing a key grounding for hypotheses on the last common ancestor of humans and monkeys and chimpanzees and the processes which guided the evolution LCA-m → LCA-c → protohumans → H. sapiens. According to a number of researchers in linguistics and artificial intelligence, the key to the meanings of expressions such as in other words, that is, and for example/for instance lies in the particular coherence relations they express in discourse. It is argued that these relations are sub-types of the relation of Elaboration and hence are ideational or semantic relations which express some experience of the world about us and within our imagination. In this paper I argue that the notion of (...) Elaboration does not provide an adequate theoretical basis for the analysis of these expressions. I show, first, that their analysis in terms of ideational relations cannot capture the way in which utterances that contain them are understood, and, second, that the phenomena which are said to fall under the relation of Elaboration do not in fact constitute a single class. I show that Sperber and Wilson's relevance theoretic pragmatics provides a better explanatory framework for the analysis of these expressions, arguing that while their notion of interpretive resemblance plays a central role in the analysis of some utterances, there are other cases which must be analysed in terms of inferential relations between propositions. In all cases, the question of how the utterance is to be understood does not depend on the classification of the coherence relation it exhibits but on how it achieves relevance. (shrink)

Politics for Derrida refers to demands for universal justice, while friendship stands in for demands to recognize the incomparable uniqueness of each person. Derrida develops the incompatibility between these demands to its furthest extreme while arguing that democracy paradoxically requires meeting the demands of both claims. The result is a democracy that is never achieved but always present only in the form of a desire for democracy. This (...) article evaluates Derrida’s position that political community is simultaneously impossible and necessary in light of similar proposals by Judith Butler and Luce Irigaray. Neil Smith has worked across the full range of the discipline of linguistics and explored its interfaces with other disciplines. In all this work he has maintained a commitment to a mentalist approach to the study of language and communication. The aim of this Special Issue is to honour his work and commitment with a collection of papers which brings together work by phonologists, syntacticians, psycholinguists, and pragmatists who share this interest in language as a central component of the human (...) mind and who have worked with Neil, whether as colleagues, collaborators, or students. Neil’s career can be viewed in relation to three main developments in modern linguistics. First, it reflects the development of generativism, in both syntax and phonology. For Neil, this has meant working within, and exploring the ramifications of, the groundbreaking theoretical framework for linguistics initiated and developed by Noam Chomsky. Neil has given full expression to this intellectual debt in two book-length studies of Chomsky’s ideas and principles (Smith and Wilson 1979, Smith 1999) and in many papers and commentaries. Notwithstanding his unswerving Chomskyan allegiance, Neil has been open to, and has encouraged, the exploration of alternative approaches to both syntax and phonology, including optimality theory, GPSG, word grammar, and categorial grammar. The second development reflected in Neil’s work is the trend towards placing research in linguistics in the context of research in cognitive psychology and philosophy of mind and language - in other words, the development of linguistics as one of the cognitive sciences, again very much a Chomskyan initiative. This ‘cognitive turn’ can be seen as, at least in part, a consequence of a commitment to generativism and to linguistic theories that aim to go beyond detailed description of data to achieve explanatory adequacy. Writing within and against the set critical practices of psychoanalytic-deconstructive-Foucauldian-feminist cultural theory, Elizabeth Wilson demonstrates, in this provocative and original book, the productivity and the pleasure of direct, complicitous engagement with the contemporary cognitive sciences. Wilson forges an eclectic method in reaction to the 'zealous but disavowed moralism' of those high cultural Theorists whose 'disciplining compulsion' concocts a monolithic picture of science in order to keep their 'sanitizing critical practice' untainted by its sinister reductionism. Her unsettling accounts of (...) texts by, for example, Karl Popper, Judith Butler, Derrida, Turing, Ebbinghaus and Freud will send many readers back to the sources. It is no surprise that such a broad and ambitious project leaves many threads loose, and no criticism at all that it succeeds more in hinting at the promise of a new connectionist politics than in offering up such a hybrid fully-formed. Privacy rights can surely be waived. But can they also be forfeited? If so, why and under what conditions? This article takes up these questions by developing a novel theory of privacy rights forfeiture that draws inspiration from Judith Thomson’s canonical work on privacy. The paper identifies two species of forfeiture rooted in modes of negligent and reckless conduct and argues that both self-directed and other-regarding considerations play a role in grounding forfeiture. The paper also contributes to the literature (...) by proposing a new taxonomy of how Hohfeldian entitlements can be divested generally. It likewise engages at length with a recent article by Benedict Rumbold and James Wilson in which the authors criticize Thomson’s work and appear to reject the very idea of privacy forfeiture. In response, I make the case that Rumbold and Wilson’s forfeiture-free model of privacy is unconvincing on substantive moral grounds. (shrink)

It describes and explains the patient-centered model examining and evaluating qualitative and quantitative research. It comprehensively covers the evolution and the six interactive components of the patient-centered clinical method, taking the reader through the relationships between the patient and doctor and the patient and clinician. All the editors are professors in the Department of Family Medicine at the University of Western Ontario, London, Canada.

This book provides answers to both normative and metaethical questions in a way that shows the interconnection of both types of questions, and also shows how a complete theory of reasons can be developed by moving back and forth between the two types of questions. It offers an account of the nature of intimate relationships and of the nature of the reasons that intimacy provides, and then uses that account to defend a traditional intuitionist metaethics. Empowerment and teamwork are buzzwords of progressive human resource practices. Along with these new job design methods come reduced hierarchical control mechanisms. In light of recent ethical scandals, there is considerable concern regarding the effectiveness of the control systems of these more recent work designs. This study compared the willingness of participants to report unethical behavior when presented with work scenarios in which the perpetrator was in the relative position of team member, peer, or subordinate and in cohesive or non-cohesive (...) conditions of relational closeness. After accounting for the covariates, both main effects and interaction effects were found, indicating that reporting behavior was influenced by relative position and relational closeness. (shrink)

Those at the forefront of the bioethics community have argued repeatedly that those doing ethics consults cannot simply be well-intentioned individuals, that some training in bioethics, group process, and facilitation is necessary to competently execute (...) a consult. As one bioethicist commented:if you approach any endeavor as an amateur activity, you will get, in the end, an amateurish version of the activity. Without a sufficient commitment of personnel, time, support, and financial resources, a healthcare organization will get the ‘ethics’ program … it set out to create: an inept, unskilled, inefficient, and highly risky ‘program’ in healthcare ethics and bioethics. To the woman, God said, “I will greatly multiply your pain in child bearing; in pain you shall bring forth children, yet your desire shall be for your husband, and he shall rule over you.”Genesis 3:16There is now a well-established body of literature documenting the pervasive inadequate treatment of pain in this country. There have also been allegations, and some data, supporting the notion that women are more likely than men to be undertreated or inappropriately diagnosed and treated for their (...) pain.One particularly troublesome study indicated that women are more likely to be given sedatives for their pain and men to be given pain medication. Speculation as to why this difference might exist has included the following: Women complain more than men; women are not accurate reporters of their pain; men are more stoic so that when they do complain of pain, “it's real”; and women are better able to tolerate pain or have better coping skills than men.
